The simplest answer is usually the correct one.
Most of the time, a human being is affixing the decals to the car during final process on the car. In an assembly plant, in the event a worker has to go take a piss break someone else has to monitor their work station. The line simply doesn't stop. Sooooo, person watching the work station for the rear hatch badges is new or inexperienced and puts the emblems on the wrong side. Every once in a while you'll also see some that aren't lined up properly or even on the wrong part of the car. There is no conspiracy theory, or design defect, it's just someone putting the stickers on wrong.
The only way to tell if the car's been in a wreck is to inspect it in person. No Carfax is ever 100% accurate.
