Question I have is...does a fuse control voltage to "relay # 30"?
I am curious as to why this relay has gone out twice in 9 months?

Have you tested the relay? Perhaps the relays are fine, there is something wrong inside the fuse block itself, where the relay connects(corrosion, burned, etc).

Relay #30 controls the blower motor, not the switches.
Fuse #10 controls the HVAC control module and the theft deterrent module.
Fuse #16 controls the instrument cluster lights, HVAC control module AND the stop lamp/cruise control release.
Fuse #4 under hood controls relay #30

My guess is that fuse #10 or #16 is loose and playing with the relay temporarily seats it.
