I remember the moon pedal with strap.. Not gonna be very effective on throttle by wire tho..

The dead man throttle strap may have possibilities..

I was in the military so I believe in redundancy and a manual backup for everything. When we went to general quarters or close maneuvering - underway replenishment or coming into port we had men stationed at the rudders so if we lost steering they could take over manually.

There will be an error code stored only IF the computer thinks something abnormal occured.

I doubt you will have a code stored because if the computer thought the throttle was falsely open, it could have cut power by shutting off fuel to the injectors.

There may have been a mechanical pedal malfunction (for example the return spring on the pedal got stuck for some reason) in which case the computer would have no way of knowing the difference between that and you keeping the pedal down on purpose.
